What Is a DOT Physical?
A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle (CMV) drivers. It verifies that a driver meets the physical and mental health standards necessary to safely operate a commercial vehicle.
The examination must be performed by a medical examiner listed on the FMCSA National Registry of Certified Medical Examiners. Upon passing, you receive a Medical Examiner's Certificate — commonly called a DOT medical card — which must be kept current throughout your CDL career.
Medical qualification decisions are made by the qualified medical examiner and applicable government agencies, not by Diesel Xtrem Pro. Requirements may vary based on your CDL class, endorsements, and individual health history.
What the examiner checks
Vision, hearing, blood pressure, cardiovascular health, neurological function, urinalysis, and overall physical fitness to operate a CMV safely.
What to bring
Photo ID, list of current medications and dosages, glasses or contacts if applicable, hearing aids if used, and any relevant medical records or specialist letters.
Medical Examiner's Certificate
A passing exam results in a DOT medical card valid for up to 24 months (shorter if health conditions require more frequent review). Keep it current — an expired card can affect your CDL status.

Preparation
What to Expect at Your DOT Physical
Medical history review
The examiner reviews your health history form, current medications, and any prior conditions that may affect your ability to safely operate a CMV.
Vision test
You must have at least 20/40 acuity in each eye (with or without correction) and adequate peripheral vision. Color recognition is also tested.
Hearing test
You must be able to perceive a forced whispered voice in the better ear at not less than 5 feet, with or without a hearing aid.
Blood pressure & cardiovascular
Blood pressure is measured and cardiovascular health is assessed. High blood pressure may result in a shorter certification period.
Urinalysis
A urine sample is collected to check for underlying medical conditions such as diabetes. This is not a drug test.
Physical examination
The examiner checks your general physical condition, neurological function, and musculoskeletal health relevant to CMV operation.
Keep Your Medical Certification Current
Your DOT medical card is typically valid for up to 24 months, but may be issued for a shorter period depending on your health status. An expired medical certificate can affect your CDL privileges. Track your expiration date in your Student Portal and set a reminder to renew well before it expires.
